ADHD Assessments $1000.00. Two payments of $500.00.
First payment is due on the day of your first virtual appointment. The second payment will be automatically invoiced 1 week later. Assessments will not continue unless payments are made.
Part 1 – Virtual visit (45-60 min.)
*Children’s assessments – PARENTS ONLY at the first visit.
The nurse practitioner will conduct a semi-formal interview, ask direct questions, inquire about symptoms, impairments, and inquire about family history.
The nurse practitioner utilizes a variety of interview skills, data collection tools, rating scales, and validated instruments. Access to a computer or tablet is required to complete some assessments.
At the conclusion of the first interview the nurse practitioner will advise you if there is sufficient information and “red flags” to proceed with the full ADHD assessment. In the event an ADHD assessment is not recommended, the assessment rate of $1000.00 will be cancelled and you will be invoiced for TIME ONLY at the NP Consultation rate of $150.00/hour.
*Children assessments require input from schoolteachers and a review of academic reports cards, you must provide all previous assessments Ie: psychologist reports, speech language reports.
*Adults seeking assessment are required to provide collateral information and evidence of symptoms and impairments. Examples of collateral informants are parents, siblings, spouse, employer, friends.
*Adults will be required to provide evidence of symptoms in childhood, such as academic report cards, and/or report from parents.
Part 2 – (60-90 min.)
The nurse practitioner will meet with you in person or virtually to discuss the results of the assessment.
Patients will be presented with education and resources as indicated.
Non-pharmacological and pharmacological treatment options will be presented.
Patients must have a primary healthcare provider willing to resume their care and treatment plan at the conclusion of the assessment.
This service is conducted by the nurse practitioner who has acquired extensive training in the assessment and diagnosis, and management of attention-deficit/hyperactivity disorder (ADHD).
The nurse practitioner cannot diagnose Autism Spectrum Disorder or Learning Disabilities.

This appointment is for patients already prescribed ADHD and psychiatric medications or those patients already diagnosed, requesting a deeper understanding of their medications and pharmacological options.
You must provide a copy of your psychological report or physician consultation notes. In most cases, you will require a primary care provider to resume your prescriptions and treatment plan.
Medication Consultations (*ADHD and psychiatric medications)
